At Level 1, also known as support linkage, included in only 2% of plans, the support coordinator plays a crucial role in aiding individuals in navigating their support plans. This level focuses on enhancing the person's understanding of their plan and establishing connections with suitable service providers. Moreover, the support coordinator provides ongoing assistance in monitoring the progress and effectiveness of the assigned plan. By offering guidance and facilitating essential connections, individuals at this level can better manage and optimize their plan for optimal outcomes.
Level 2: Support Coordination
Level 2: Coordination of supports focuses on collaboration and understanding to ensure that participants receive the best possible care. Support coordinators are dedicated to working closely with participants to grasp their funding requirements and service expectations. By emphasizing communication and cooperation, support coordinators guide participants in forming and nurturing a network of both formal and informal supports. Through this level, participants can tailor their care plans to suit their individual needs and preferences effectively.
